About Grace in Care

Grace in Care provides registered nurse-led dementia care support across Adelaide for individuals, families and carers. We offer clinical consultations, in-home dementia care, and dementia education delivered with compassion and professional expertise.

Grace in Care exists because of the families who shaped us; the mothers, fathers, grandmothers, and grandfathers whose ageing journeys taught us what truly matters: compassion, patience, and dignity.
We're a family-led service, founded by Carrol and Jody, who have worked side by side in nursing and aged care for over 30 years. Throughout our careers, we've had the privilege of supporting older adults and their families across aged care, acute care, community health, disability, palliative care, and dementia services.

Together with our dementia-trained team, we bring not just experience and professionalism, but genuine care and understanding to every family we support.

Dementia care support at every stage

Grace in Care provides dementia care support that grows and adapts alongside you. We're here to help you understand what's happening, what might lie ahead, and how to respond with confidence, compassion, and care.

In the early stages, our support often focuses on making sense of the changes you're seeing. We can talk through your concerns, help you prepare for GP visits and assessments, and guide you through the diagnosis process. Through consultation, education, and care planning, we help you and your family feel more informed and ready for what comes next.

As dementia progresses, the focus naturally shifts. We're here to provide practical in-home care, help maintain familiar routines, support meaningful communication, and stand beside carers who are doing their best each day. It may be managing safety concerns, understanding behaviour changes, or meeting growing care needs, and we offer calm, person-centred guidance and hands-on support whenever it's needed.

Grace in Care approach to dementia care support

Grace in Care provides dementia support that's guided by clinical knowledge, shaped by lived experience, and delivered with genuine care and respect. All our services include registered nurse oversight, ensuring you have informed support for decision-making as needs evolve.

Our approach puts the person first, always. Care is tailored around individual routines, preferences, and the relationships that matter, because we know that dementia affects each person and family in their own unique way. We work calmly and thoughtfully, taking the time to truly listen, explain things clearly, and respond with care.

Where it's needed, our registered nurses provide clinical monitoring and short-term nursing support alongside dementia care and family guidance.

Clinical insight within whole-person dementia care

Our services are delivered with registered nurse oversight, which means we're able to care for the brain, the body, and your loved one's emotional wellbeing; all together, as they should be. This whole-person approach helps us spot physical, cognitive, and behavioural changes early, which is so important for people living with dementia.

We keep a close eye on things like nutrition and hydration, mobility, continence, medication effects, and signs of infection. All factors that can significantly impact wellbeing and cognitive function.

When clinically needed, our services can also include short-term nursing support, such as wound care. This helps prevent deterioration and avoid unnecessary hospital admissions, while always maintaining our strong focus on dementia-specific care and support for your family.

Support with navigating My Aged Care and GP appointments

Grace in Care offers experienced, registered nurse-led support to help you make sense of it all. This might include attending GP or specialist appointments with you, supporting you through phone or case conferences, helping with follow-up, and explaining your options in clear, practical terms. By helping families understand available services, eligibility, and what comes next, we take some of the stress out of an already challenging time and support you in making informed decisions with confidence.

Understanding dementia care support and why it is needed

Dementia is not a single condition. It's a broad term that describes the changes in memory, thinking, behaviour, and daily life that unfold gradually over time. Each person's experience is unique, and no two journeys are quite the same.

For many families, the early signs can feel uncertain. Perhaps you've noticed memory lapses, shifts in mood or behaviour, or your loved one struggling with tasks that once came easily. These moments can stir worry and questions, often long before any diagnosis brings clarity.

As dementia progresses, its reach extends beyond the person living with it. Family members and carers find themselves learning to adapt, navigating difficult decisions, and providing more support with each passing day. It can feel overwhelming, but you don't have to face it alone. Grace in Care is here to walk alongside you and your loved ones.
